Accreditation Maintenance

Even after accreditation, the CB remains under on-going oversight by the management office through continuous supervision and monitoring.
This ensures reliable and consistent operations, adherence to technical and ethical standards, and a framework for continuous improvement.

Accreditation is maintained through:

  • Annual on‑site office assessment.
  • Witness audits based on audit volume and risk.
  • Annual reporting of key performance indicators.
  • Real‑time data submission through the ASQMS supervisory portal.

If performance thresholds are not met, oversight and monitoring may be strengthened or the status may be converted to conditional accreditation.

Accreditation may be suspended or revoked in the following cases:

  • Nonconformities are not resolved within the specified timeframe.
  • Key performance repeatedly falls below threshold levels.
  • False statements or breaches of ethics have been identified

Appeals must be submitted using the ASQMS form within 30 days of the notice of suspension or revocation.
